Automotive Claims

The following information would be helpful in processing your automotive claim:

  1. Date of loss
  2. Time of loss
  3. Location of accident (street, direction, town, and state)
  4. Vehicle you were driving
  5. Name of individual driving your vehicle
  6. Description of damage to your vehicle
  7. Is vehicle driveable?
  8. Description of injuries (if any)
  9. Description of what happened
  10. Were the police called? If so, retain police report number and name of police department)
  11. Write down name(s) and phone numbers of any witnesses
  12. Other driver's information
    • Name, address & phone number(s)
    • Driver's License number of other driver
    • Description of injuries (if any)
    • Type of vehicle (year, make, model, color, plate number)
    • Insurance carrier and policy number
    • Description of damage to vehicle
    • Is vehicle driveable?
